The healthcare industry is shifting from a volume-driven to a value-driven system, but how do we measure value? What information do we need to do a comprehensive benefit/cost analysis? What information should we be tracking at the design stage to allow for this assessment at construction and post-occupancy?

The Academy of Architecture for Health Foundation has awarded a $20,000 grant to support the study of the value analysis of lean processes in the design and integrated project delivery. “Integrated Project Delivery (IPD) and lean methodologies are increasingly being utilized for the design and construction of complex buildings, such as healthcare facilities,” said Upali Nanda, vice president and director of research for CADRE. “Although case study presentations are becoming commonplace, in-depth analyses of the value to the organization, and potential benefit/cost or return on investment analyses remain challenging to compile.”

To meet this challenge, HKS’ research entity, CADRE, Akron Children’s Hospital and Texas A&M University will come together. Dr. Upali Nanda from CADRE and Dr. Zofia Rybkowski, assistant professor in the department of construction science at Texas A&M, will be co-principal investigators on this grant, working with an accomplished team of experts and graduate students.

The team’s research will ultimately define explicit benefits and costs of IPD.  In this first phase of a multi-phase study, researchers will gather information from key stakeholders of a lean-IPD project about perceived benefits and costs and develop an inventory of metrics currently being tracked. The goal is to develop a framework of recommended metrics that will facilitate preparation of systematic benefit/cost analyses in the future.

According to Nanda and Rybkowski, the research is significant. “It will develop an inventory of metrics from a real-life project, and a framework to understand the fiscal implications of a value-driven approach to design and project delivery,” said Nanda. “This will allow all stakeholders in general, and architects in particular, to systematically collect data during design, that can allow a rigorous benefit/cost analysis in the future.”

Integrated Project Delivery is a project delivery approach that integrates people, systems, business structures and practices into a process that collaboratively harnesses the talents and insights of all participants to optimize project results, increase value to the owner, reduce waste and maximize efficiency through all phases of design, fabrication and construction. Lean design integrates operational continuous process improvement with the design process to optimize use of staff and facility resources as well as to innovate in the facility component of healthcare delivery. Lean construction is a project delivery process that uses continuous improvement to eliminate waste and add value, within a culture of respect, in order to optimize time, cost, quality, safety and morale.The lean ideal is minimum waste and maximum value.

The grant is awarded through the Academy of Architecture for Health Foundation (AAHF). The mission of the group is to enhance the knowledge and effectiveness of those who create healthcare environments through the funding of critical educational and research activities. AAHF also supports the AIA Knowledge Community, the Academy of Architecture for Health, with management of its funds.
